Citizenship Clinic

Intake form used to obtain information and to determine eligibility for the Adoptees United Citizenship Clinic. Please complete it as best as you can.

Intake

Submission of information through this online form is considered confidential. Submission of information does not create an attorney-client relationship between you and me or Adoptee Rights Law Center PLLC. As a matter of strict policy, Adoptee Rights Law Center and its clinic does not agree to represent clients without first securing a signed retainer agreement.

Current Name(Required)
Birth/Initial Name (if different/known)
Sometimes a name for an adoptee can be complicated. If you need to explain or provide more context, please do.
Current Address(Required)
I may need this to send notices or other correspondence and in some cases to determine jurisdiction over legal matters. It is not shared with anyone unless I have your permission.
I may need this to get in touch with you.
This is important for issues related to the Child Citizenship Act. Again, it is confidential information that I do not share.
MM slash DD slash YYYY
Have you ever been married?
This is a question asked on most immigration forms.
Current Marital Status
What is your current marital status?

Hang On.

If you were born in the United States you do not qualify for the clinic at this time. Please review your answer to ensure it is correct.

Was your adoption finalized in your country of birth?
This would mean that your adoptive parents completed the adoption in your country of birth and likely changed your name as part of that adoption
Was your adoption finalized in the United States?
This would mean that adoptive parents filed a court case in the United States to finalize the adoption in the U.S.
This is almost always the state where your adoptive parents lived
Father/Parent Name
Your adoptive father or parent’s name
Mother/Parent Name
Your adoptive mother or second parent’s name
Sometimes called a “maiden” name, if applicable
Do You Have an "A Number"
An A number, or Alien Registration number, is assigned to all people who immigrate to the United States, even as children. If you have a green card (legal permanent resident card) it will be listed on that.
If you have an A Number, let me know what it is. I may need it later if I represent you.
Have you ever had a US passport, whether current or expired?
Please provide as much detail as you feel necessary to let me know what you are hoping to find or resolve through legal help
Check what you think you may need for legal services
This helps us understand more about what you need. Don’t worry if you do not know.
Acknowledgment(Required)
I understand and agree that Adoptee Rights Law Center is providing free and low cost legal services to intercountry adoptees who have issues or questions about US citizenship, through a service funded by Adoptees United Inc. I am asking to be considered for this service. I understand that Gregory Luce is the attorney who will review my intake and determine if I qualify for the Citizenship Clinic.
