Intake Information

Thanks for your interest in securing legal help or advice. Please let me know more about your issue so that I can determine if I can help. I also use this information to determine how best to refer individuals whom I cannot represent (for example, your issue is one that does not involve federal immigration law or is related to state law outside of Minnesota). I cannot guarantee, however, that I can successfully refer people to an attorney.

Please understand that I consider submission of information on this website form as confidential, but submission of information does not create an attorney-client relationship between you and me or Adoptee Rights Law Center PLLC. As a matter of strict policy, I will not agree to represent a new client without first investigating for possible conflicts of interest and also securing a retainer or engagement agreement that is signed by you and by me on behalf of Adoptee Rights Law Center.

  • Sometimes a name for an adoptee can be complicated. If you need to explain or provide more context, please do.
  • I may need this to send notices or other correspondence and in some cases to determine jurisdiction over legal matters. It is not shared with anyone unless I have your permission.
  • I need this to get in touch with you at times
  • Date Format: MM slash DD slash YYYY
  • This would mean that adoptive parents filed a court case in the United States to finalize the adoption in the U.S.
  • Where your adoption was finalized. For those born in the United States, this is often, but not always, the same as the state of birth.
  • If you have an A Number (Alien Registration Number) it is important for me to have that
  • Please indicate if your issue involves any of the following (check as many as apply):
  • Please provide as much detail as you feel necessary to let me know what you are hoping to find or resolve through legal help
  • This field is for validation purposes and should be left unchanged.
